Request Account Ownership
What documents should I attach?
If your account was registered in the name of an individual person, you will need to provide a color copy of that individual’s Drivers License or State ID.
If the account had a company name listed during registration, you will need to submit a copy of one of the following documents as proof of ownership.
- Business License
- Certificate of Association
- 501(c)(3)
- DBA/ Fictitious Name Documentation
What information should be on the documents?
- Individual/Company Name
- Street Address matching the address on the account
